We have enhanced the Survey Questionnaire module to allow respondents to review and evaluate multiple directors within a single survey. 


With this update, respondents do not need to complete separate surveys for each director, saving time and effort. This also reduces administrative workload while ensuring a consistent review format.


To create a survey form:


1) Log in to your Board.Vision account using your registered email address and password. 

 

2) Once you are logged in, click on Survey Questionnaire from the left menu.  

 

3) Choose a Template or Create a New Survey

  • You can go to the Templates section to use a pre-existing survey template.
  • Alternatively, click on the +Add Survey button to create a new survey from scratch.

 

4) On the "Add Survey" page, enter the required Survey Name.


You may provide a brief description explaining the purpose of the survey and how long it will take to complete. This sets expectations for respondents. 


5) Under "Settings", if needed, set a deadline.


6) Next, select the “Allow recipient to edit their submission” checkbox.


Note:

a) If no deadline is set, recipients can edit his/her submission at any time.

b) If a deadline is set, recipients can edit his/her submission until the deadline.


7) Scroll down to the "Add Recipients" section, then:


a) Enter the recipient's email address, or

b) Click the Add BV Usebutton to select a recipient from the Board.Vision users list. Once selected, click Confirm.

 

8) Once you have entered all the required fields, click on the Next button at the bottom of your screen.


9) You will be directed to the "Create Survey" page. Add Questions and select appropriate question types (e.g., rating scale, multiple choice, text input). 


10) Add a Dynamic Panel for Multiple Reviews

  • Navigate to the "Containers" section.
  • Click on Dynamic Panel and drag it to the desired location on the page.
  • This will allow respondents to review multiple directors within the same survey.

 


11) Add Questions to the Dynamic Panel

  • A panel will appear in the selected section.
  • Add relevant questions within the panel.
  • Choose the appropriate question types (e.g., rating scale, multiple choice, text input) to structure the evaluation.



12) Activate and Send the Survey

  • After adding all questions and configuring the settings, go to "Preview" to review the survey.
  • Click on "Activate" to finalise and send it out to respondents.


13) Notify Recipients

  • When the Activate Survey pop-up screen appears, check the "Notify Recipient" checkbox.
  • Click Confirm to send the survey to the recipients.




Upon responding to the Survey:


Respondents will encounter a dynamic panel within the survey, where they can click "Add New" at the bottom of the screen to add a new panel for each director they need to evaluate.



Once added a new panel, they can enter the name of Director  into the provided field.
